Identification of surgical patients at high risk of OSAS using the Berlin Questionnaire to detect potentialhigh risk of adverse respiratory events in post anesthesia care unit

摘要:

Obstructive sleep apnea syndrome (OSAS) increases the risk of post-surgery complications. This study uses Berlin Questionnaire (BQ) to identify Chinese adult surgical patients who are at a high risk of OSAS and to determine if the BQ could be used to detect potential high risk of adverse respiratory events in the post anesthesia care unit (PACU). Results indicated that only 11.4% of the patients were considered at a high risk of OSAS. Age and body mass index are the key factors for the risk of OSAS prevalence in China and also gender specific. Furthermore, the incidence of adverse respiratory events in the PACU was higher in patients with high risk of OSAS than others (6.8% vs. 0.9%, <0.001). They also stayed longer than others in the PACU (95±28 min vs. 62±19 min, <0.001). Age, high risk for OSAS, and smoking were independent risk factors for the occurrence of adverse respiratory events in the PACU. The BQ may be adopted as a screening tool for anesthesiologists in China to identify patients who are at high risk of OSAS and determine the potential risk of developing postoperative respiratory complications in the PACU.

Risks of modular integrated construction: A review and future research directions

摘要: Stakeholders remain skeptical in adopting modular integrated construction (MiC) because of the associated risks and uncertainties, although its benefits have been extensively documented. The unique business model of MiC nurtures several risks and uncertainties different from those of the conventional construction approach. Despite the growing attention on MiC with its market expansion, no systematic evaluation is in place to monitor its risks research progress. Accordingly, this research reviewed published literature addressing the risks associated with MiC from 1992 to 2019. Analysis reveals that the research publications on risks of MiC witnessed a steady growth, with considerable progress occurring in the last decade. Result implies that the risk of MiC has gained extra attention in the construction engineering and management domain in recent times. Existing empirical studies have focused heavily on perceived implementation risks, supply chain risks, schedule risks, investment risks, structural risks, ergonomic risks, and MiC risk management strategies, which indicate that MiC is associated with a host of risk events. The research further identified the critical risk events (CREs) in the application of MiC based on frequency of occurrence. The identified CREs contributes to the checklists of risk events in the implementation of offsite construction (OSC). The latter may be useful in risk planning, especially where the MiC is less developed, and fewer or no bespoke risk assessment exists. Research gaps in existing studies are highlighted in this research, and areas for further studies are then proposed. Thus, it makes a useful contribution to the scholarly literature on the risk of OSC and may prove useful to offsite construction researchers, industry practitioners, and project managers.

Smoothing ramp events in wind farm based on dynamic programming in energy internet

摘要:

The concept of energy internet has been gradually accepted, which can optimize the consumption of fossil energy and renewable energy resources. When wind power is integrated into the main grid, ramp events caused by stochastic wind power fluctuation may threaten the security of power systems. This paper proposes a dynamic programming method in smoothing ramp events. First, the energy internet model of wind power, pumped storage power station, and gas power station is established. Then, the optimization problem in the energy internet is transformed into a multi-stage dynamic programming problem, and the dynamic programming method proposed is applied to solve the optimization problem. Finally, the evaluation functions are introduced to evaluate pollutant emissions. The results show that the dynamic programming method proposed is effective for smoothing wind power and reducing ramp events in energy internet.

The effects of festivals and special events on city image design

摘要: City marketing provides a good vehicle to strengthen a city in competition. On the analysis of the important roles of events and city image in the context of city marketing, this paper suggests that holding festivals or special events is an effective approach to enhance city marketing. The positive impacts of festivals and special events, such as the identification of city image, the promotion of the transmission of city image, and the reversal of the negative image of the city, are discussed. The negative impacts of festivals and special events are also talked about. The unique image of the city could be identified by the City Mind Identity, the City Visual Identity and the City Behavior Identity concerned with festivals and special events.

Advances on immune-related adverse events associated with immune checkpoint inhibitors

摘要: Immunotherapy has recently led to a paradigm shift in cancer therapy, in which immune checkpoint inhibitors (ICIs) are the most successful agents approved for multiple advanced malignancies. However, given the nature of the non-specific activation of effector T cells, ICIs are remarkably associated with a substantial risk of immune-related adverse events (irAEs) in almost all organs or systems. Up to 90% of patients who received ICIs combination therapy experienced irAEs, of which majority were low-grade toxicity. Cytotoxic lymphocyte antigen-4 and programmed cell death protein-1/programmed cell death ligand 1 inhibitors usually display distinct features of irAEs. In this review, the mechanisms of action of ICIs and how they may cause irAEs are described. Some unsolved challenges, however really engrossing issues, such as the association between irAEs and cancer treatment response, tumor response to irAEs therapy, and ICIs in challenging populations, are comprehensively summarized.

Key uncertainty events impacting on the completion time of highway construction projects

摘要: This paper examines the uncertainty events encountered in the process of constructing highways, and evaluates their impact on construction time, on highway projects in South Africa. The rationale for this examination stems from the view held by scholars that the construction of highways is a complex process, taking place in changing environments and often beset by uncertainties; and that there is a lack of appropriate evaluation of these uncertainty events occurring during the construction process. The research made use of a review of extant literature in the area of uncertainty management, and modeling in infrastructure projects, to guide the direction of the study. The inquiry process consisted of brainstorming by highway experts and interviewing them to identify the uncertainty factors that impact construction time. An uncertainty matrix for South African highway projects was developed, using a quantitative model and descriptive statistics. It emerged from the study that the uncertainty events affecting the construction time of highway projects are distributed across economic, environmental, financial, legal, political, social and technical factors. Also, it was found that each factor might account for several uncertainty events which impact on construction time differently, through a combination of the uncertainty events of the individual construction activities. Based on the obtained data, an Adaptive Neuro Fuzzy Inference System (ANFIS) has been developed, as a simple, reliable and accurate advanced machine learning technique to assess the impact of uncertainty events on the completion time of highway construction projects. To validate the ANFIS model, the Stepwise Regression (SR) models have been designed and their results are compared with the results of the ANFIS. Based on the predicted impact size of uncertainty events on the time of highway projects, it can be concluded that construction time on South African highway projects is significantly related to the social and technical uncertainties factors.

evolution of charged and neutral nanoparticle concentrations during atmospheric new particle formation events

摘要: Over 100 new particle formation events were studied. In 50 events, charged and neutral particles were not formed at the same time. In 42 of these events the charged particles formed before the neutral particles. Their subsequent growth rates were not determined by the particle charge. The result suggests that ion induced nucleation plays a role in particle formation. Time series of nanoparticle number concentration during new particle formation (NPF) events in the urban environment of Brisbane, Australia, showed that the formation of charged particles often occurred before that of neutral particles. We monitored 241 days during the calendar year 2012 over which NPF events were observed on 108 days. We studied the times at which the charged and neutral particle concentrations in the size range 1.8–3.2 nm reached their peak values and found that they were clearly different in 50 events with the peak neutral particle concentration lagging behind the charged particle concentration during 42 of these events with a mean time lag of 24±12 min. While the charged particles were more likely to form before the neutral particles, once formed, the growth rate of the particles did not depend on their charge. While ion-induced nucleation is not the dominant mechanism of NPF in the atmosphere, our observations suggest that the presence of ions in the atmosphere plays a role that cannot be ignored.

Key features of new particle formation events at background sites in China and their influence on cloud

摘要: New particle formation (NPF) event at multi rural sites in China Identifying the characteristics of NPF event Comparing NPF event between clean and polluted conditions Quantifying contribution to the cloud condensation nuclei Implication of climate and air quality Long-term continuous measurements of particle number size distributions with mobility diameter sizes ranging from 3 to 800 nm were performed to study new particle formation (NPF) events at Shangdianzi (SDZ), Mt. Tai (TS), and Lin’an (LAN) stations representing the background atmospheric conditions in the North China Plain (NCP), Central East China (CEC), and Yangtze River Delta (YRD) regions, respectively. The mean formation rate of 3-nm particles was 6.3, 3.7, and 5.8 cm ·s , and the mean particle growth rate was 3.6, 6.0, and 6.2 nm·h at SDZ, TS, and LAN, respectively. The NPF event characteristics at the three sites indicate that there may be a stronger source of low volatile vapors and higher condensational sink of pre-existing particles in the YRD region. The formation rate of NPF events at these sites, as well as the condensation sink, is approximately 10 times higher than some results reported at rural/urban sites in western countries. However, the growth rates appear to be 1–2 times higher. Approximately 12%–17% of all NPF events with nucleated particles grow to a climate-relevant size (>50 nm). These kinds of NPF events were normally observed with higher growth rate than the other NPF cases. Generally, the cloud condensation nuclei (CCN) number concentration can be enhanced by approximately a factor of 2–6 on these event days. The mean value of the enhancement factor is lowest at LAN (2–3) and highest at SDZ (~4). NPF events have also been found to have greater impact on CCN production in China at the regional scale than in the other background sites worldwide.

华北地区持续性强霾事件发生的环境与动力条件

本文是近期针对华北地区霾的长期变化特征持续强霾事件发生的环境和动力条件方面研究的个结。华北地区近50 年来霾日呈现明显的上升趋势,尤其是进入21 世纪以来,霾日增加明显同时,持续性霾事件的发生频次也呈现类似的上升趋势华北地区霾污染事件频发可以归因于不利的气象环境条件和人为排放的大量污染物的共同作用。气象环境条件方面主要表现为:在气变暖的大背景下,华北地区冬季气温的持续上升、地面风速的减小以及大气稳定度的加强均对霾日的增加有显著影响。厄尔尼诺(El Nino) 年冬季,东亚大槽偏弱,冷空气活动弱,华北地区盛行异常偏南风,有利于霾天气的形成;而拉尼娜(La Nina) 年冬季,东亚大槽偏强,冷空气活动强,华北地区盛行异常偏北风,不利于霾天气的形成华北地区持续强霾事件主要发生在平直西风型和高压脊型两类环流形势下,对流层中低层为持续深厚的下沉气流,使大气边界层厚度减少污染物扩散能力减弱,有利于霾天气的维持和加剧。

气候变暖背景下的极端天气气候事件与防灾减灾

摘要:

首先概括极端天气气候事件以及“气候极值”的相关定义,并把极端事件分为单要素的极端事件、与天气现象有关的极端事件、多要素极端事件和极端气候事件。在此基础上,总结上述几类极端事件在气候变暖背景下的变化趋势及影响。指出气候变暖背景下我国长江中下游区域强降水事件更趋频繁,我国东部地区高温热浪天气更为明显;东北华北地区干旱趋势增加,尤其在20世纪末期和21世纪初期最为明显;近10年来西南地区干旱频繁发生。为减轻日益增加的重大气象灾害的损失,我国有必要加强高影响极端事件的监测、预警能力建设,同时还必须根据极端天气气候事件变化规律加强工程性防御措施,以防范和应对强降水引发的洪涝灾害和城市渍涝,以及与降水持续不足有关的重大干旱和高温热浪等气象灾害。

Understanding and addressing the environmental risk of microplastics

Over the past decades, the plastic production has been dramatically increased. Indeed, a category of small plastic particles mainly with the shapes of fragments, fibers, or spheres, called microplastics (particles smaller than 5 mm) and nanoplastics (particles smaller than 1 μm) have attracted particular attention. Because of its wide distribution in the environment and potential adverse effects to animal and human, microplastic pollution has been reported as a serious environment problem receiving increased attention in recent years. As one of the commonly detected emerging contaminants in the environment, recent evidence indicates that the concentration of microplastics show an increasing trend, for the reason that up to 12.7 million metric tons of plastic litter is released into aquatic environment from land-based sources each year. Furthermore, microplastic exposure levels of model organisms in laboratory studies are usually several orders of magnitude higher than those found in environment, and the microplastics exposure conditions are also different with those observed in the environment. Additionally, the detection of microplastics in feces indicates that they can be excreted out of the bodies of animal and human. Hence, great uncertainties might exist in microplastics exposure and health risk assessment based on current studies, which might be exaggerated. Policies reduce microplastic emission sources and hence minimize their environmental risks are determined. To promote the above policies, we must first overcome the technical obstacles of detecting microplastics in various samples.

Ecological Risk Management of Drinking Water Project: The Case Study of Kunming City

摘要: Following rapid infrastructure development and industrialization, the problems of water pollution and water shortage have become more severe. Whether there is safe drinking water in cities has attracted wide attention. The ecological risk management of drinking water project is an important means of ensuring the safety of a drinking water source. Based on ecological risk assessment and management theories, this paper establishes an ecological risk management model and assessment system with the aim of providing theoretical guidance and scientific basis for formulating a policy on the safety and protection of drinking water sources in a city. Kunming is one of the cities plagued by severe water shortage in China. Its ecological risk management of drinking water has attracted the attention of both the local government and the public. Using Kunming as the case study, this paper conducts a comparative analysis and assessment on three major reservoirs that face ecological risks. It highlights the existing problems and gives helpful suggestions.
